Our Services

Horizons Community Care offers private-duty nursing and care for chronic and acute illnesses and nursing/caregiving for pediatric and adult patients with disabilities. Our nursing staff is available for care 24 hours a day, 7 days a week. A calling agency is also available for assistance or direction to prevent hospitalization. Our services are 100% covered by Medicaid, Indiana Aged and Disabled Waiver, and Choice. Private payment is also accepted.
